LifeNoW Transition Support Worker
LifeNoW are working in partnership with Derian House to provide this exciting new opportunity. The post holder will work alongside the team at Derian house Children’s Hospice with an identified group of young adults aged 24-28 years to facilitate transition from the support networks provided by the Hospice following discharge from these services.
The main purpose of this post is to work with young adults with a life-limiting condition to offer guidance and support for transition from children’s hospice services, to raise aspirations and help remove barriers and also to develop transition pathways and services which will promote autonomy and independence.
The post is based at Derian House Children’s Hospice in Chorley and will work as part of the multi-disciplinary team within the Hospice. For further details about these positions or to arrange an informal discussion please contact a member of staff from the Senior Care Team on 01257 233300.
